STEP 1—Fill out Registration Form

STEP 2—SKIP IF YOU HAVE EMPLOYEES 


If you do not have any employees other than yourself and you want to opt out of having Worker’s Compensation insurance on yourself, affidavit of no employees form must be filled out.  CAUTION an injury on the job could be denied by your private health insurance because it is a workplace injury.

STEP 3—Choose the appropriate bond form based on how your company is organized. Download the form and provide that form to your insurance agent to be filled out. You MUST use this Bond form. You will need a copy of the bond. Your insurance company will send the bond directly to the state.

STEP 4 Obtain Certificate of Insurance  


the irrigation contractor carries general liability insurance with a minimum of $100,000 per occurrence, bodily injury insurance with a minimum of $300,000 per occurrence, property damage insurance with a minimum of $50,000, and worker's compensation insurance with a minimum of $500,000



STEP 5 Plumber of Record


You must have  a Illinois licensed plumber as Plumber of Record. This person can be an employee of your company (paid on your payroll) or you can hire an Plumbing company with licensed plumbers.  Provide a copy of the plumbers license and if you are hiring a company provide a copy of their Plumbing registration and a “Letter of Intent” stating that they will be your “Plumber of Record”


STEP 6 Employee that has taken past the 16 hour class & test


Provide a copy of the class Certificate that an employee has taken & passed a 16 hour Irrigation registration class and copies of “4 hour” continuation education certificates(s) if applicable.


STEP 7 Check for $400- $1500 based on the number of anticipated irrigation employees
