Irrigation employee registration instructions & form
After you have received your company registration certificate, then you can proceed with registering your employees. Do not attempt to register your company and your employees at the same time.
STEP 1
Fill out a form for every employee working, designing, or supervising irrigation projects

STEP 2
Copy of your Irrigation contractor registration certificate
STEP 3
Mail all of the above to IDPH
If an employee quits or is fired prior to December 31 then you must fill out the cancellation form and pay Cancellation fee. Otherwise, that employee will not be able to register as an employee for other Irrigation company.
